What should you bring to an appointment?
Bring any medication bottles, vitamins, herbs, and supplements that are taken.
Identification to prove who you are, such as driver's licenses or passport.
All identification needs to be current or the office providers may decline to see patient due to fraud, stolen identity, or legal issues.
It is recommend that patients keep a list of medications with their identification in wallets and purses in case of emergencies.
